Total value of services offered in advertised consultation is $21. X-ray included in consultation is billed as CPT 72082 (2 or 3 views of the spine) and is valued at $11. Examination included in consultation is billed as CPT 99201 (problem focused examination) and is valued at $10. The value of the advertised services represent my usual, reasonable and customary charges for these services. My complete fee schedule is available to patients upon request. If insurance is billed, I am required to collect all applicable copayments and deductibles.
